  To create a database and have it work with your script -- follow 3 steps below...
  
  STEP 1: Create a new database. Name it something short.
  
  STEP 2: Create a database username. We recommend entering the same name as you did in step 1. It'll make it less confusing when configuring your script.
  
  STEP 3: Select both from the drop down-field, and click on "Assign Privileges".
  
  Done! You can now enter your DB name, DB username, and your DB username pass into your script.
  Your account MySQL is DISABLED.
  Click here to Enable NOW! (Then choose "Turn ON MySQL Database Support" option.)
